Advance Care Planning and Dementia at Home - Research Survey

Advance Care Planning Australia has initiated a research project to better understand the experiences of people living with dementia, and people who care for people living with dementia, to help plan for future care.

The survey is designed for people who:

  • Have received diagnosis of dementia and are living at home (not in a residential care home).
  • Are providing care, or have provided care within the past 12 months, for someone living at home (not in a residential care home) who has received a diagnosis of dementia

This research is being conducted by Advance Care Planning Australia and is supported by Alzheimer’s Australia. All responses are confidential.
